What forms of payment are accepted?

All forms of credit and debit cards including Visa, Mastercard, Discover and American Express.

When can I start using my membership?

Your membership is effective immediately!

How do I schedule my preventive care?

Simply call our office and set up an appointment for yourself or your family members.!

How can I use my Member Exclusive discount for restorative, elective or cosmetic treatment?

Simply schedule an appointment with our office and be sure to tell us you’re a member. When you arrive, we’ll make sure your membership is “current” and you will be eligible for instant savings off our regular fees.

How do I know if I need Perio membership?

If you’re a current patient, please contact us to confirm you’re on Perio Maintenance. If you are a new patient, we will make a recommendation after your initial exam.

What happens if I miss an appointment?

It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last minute cancellations.

Can I add members to my account?

Absolutely! Your entire family can join.

Can I transfer my membership to another dentist?

Your club membership is solely with our office and as such is non-transferrable. If you move to another area, you may cancel your membership at any time.

How can I check on the status of my account?

Login to your Member Portal and look under Account Members. You’ll see the status of each member above his/her name.

Can I change or update my billing information?

Yes! Simply login to your Member Portal and click “Edit Your Payment Method”.

How do I cancel my membership?

Login to your Member Portal and click the “Remove” button for each member you’d like to cancel.

If I cancel, can I get a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.

How much do I pay to sign up for my club membership?

You’ll pay only the Lifetime Activation fee to join. This fee includes your first month’s membership dues.

Can I prepay my membership fees?

Memberships are currently month-to-month only.

What information is required for me to sign up for my club membership?

Your Dentists in Flagstaff

Dr. Madison Kurz

Dr. Kurz was born in Anchorage, Alaska, and grew up living abroad. She completed her undergraduate studies in Physiology and Molecular & Cellular Biology at the University of Arizona. Afterward, Dr. Kurz attended Dental School at Roseman University of Health Sciences in Salt Lake City, Utah. Dr. Kurz is affiliated with the American Dental Association, the Arizona Dental Association, the Northern Arizona Dental Association, and the Academy of General Dentistry, and volunteers with Arizona Mission of Mercy. She has pursued continuing education in full mouth reconstruction, Invisalign, CEREC, and injectable facial esthetics. While Dr. Kurz enjoys all aspects of dentistry, she has a special affinity for cosmetic dentistry and Invisalign. She enjoys getting to know and learning new things from her patients. In her spare time, she enjoys skiing, snowboarding, hiking, running, biking, and camping.

Dr. Kevin Chittenden

Dr. Kevin is originally from Snohomish, Washington. He attended Washington State University and earned a degree in Zoology. He then earned his dental degree from Roseman University in Salt Lake City, Utah. He is affiliated with the American Dental Association, Arizona Dental Association, The Academy of General Dentistry and Northern Arizona Dental Association. Dr. Kevin is also a volunteer at the local Poore Medical Clinic and Arizona Mission of Mercy. Dr. Kevin has pursued advanced education in sleep appliances, implant placement, root canal therapy, and guided surgery.

Dr. Kevin enjoys all aspects of dentistry, with a special affinity for oral surgery and implants. He enjoys talking to his patients about their favorite sports or outdoor adventures. While he grew up playing a variety of different sports, Dr. Kevin still plays tennis and soccer regularly. He joined the rowing team when attending Washington State University and raced in the top boat achieving 6th in the nation. Around Flagstaff, you can often find him running a disc-golf course, playing pickleball, or skiing the slopes.

Michele is an incredibly talented RDH that graduated with her B.S. in Hygiene from Loma Linda University. She has over 10 years of experience and an encyclopedic depth of knowledge on the topic of dental care, cats and Waterpiks! Her goals are to continue living her passions including traveling to the best beaches on earth, health, fitness and Handles Icecream. We're lucky to have such an amazing person at VS.
